Before You Start

Verify patient name matches the encounter

Position device between you and patient (table/desk works well)

Check microphone/computer isn't blocked by objects

Keep Knowtex app open in the tab during entire recording (you can switch tabs)

During the Visit

🎯 Work Out Loud

Knowtex only summarizes what it hears. Verbalize key information:

Patient Status

  • "I’m seeing you as a new patient" or "This is a followup visit for [condition]"
  • "Last seen [timeframe] for [reason]"

Physical Exam Findings

  • State findings as you examine: "Lungs clear bilaterally"
  • Call out vitals: "Blood pressure 145 over 90"
  • Specify laterality: "Tenderness in the right shoulder"
  • Narrate visual cues: "Patient is pointing to the left lower quadrant"

Labs & Imaging

  • Read specific values: "Hemoglobin 11.2, creatinine 1.4"
  • State key findings: "Chest X-ray shows right lower lobe infiltrate"

🎤 Optimize Audio Quality

  • Minimize interruptions - Let each person finish speaking
  • Reduce background noise - Close doors/windows when possible
  • Echo unclear statements - "So you're saying the pain is 7 out of 10?"
  • Speak at normal pace - No need to slow down artificially

📋 Structure Your Assessment & Plan

Use trigger phrases the AI recognizes:

Diagnosis

  • "My assessment is..."
  • "I'm diagnosing you with..."
  • "This appears to be..."

Treatment Plan

  • "The treatment plan includes..."
  • "I'm prescribing..."
  • "We'll order..."

Follow-up

  • "Return in 2 weeks"
  • "Follow up in 3 months"
  • "Schedule with me within 30 days"

After the Visit

Quick Post-Visit Dictation

Missed something? Need to add sensitive details?

  1. Stop recording after patient leaves
  2. Upload initial recording
  3. Add separate dictation for:
  1. MDM complexity details
  2. Differential diagnosis reasoning
  3. Sensitive psychosocial factors
  4. Additional billing elements

Common Pitfalls to Avoid

❌ Mumbling exam findings to yourself

❌ Using only gestures without verbal description

❌ Saying "normal exam" without specifics

❌ Forgetting to state follow-up timeline

❌ Closing app before uploading the audio

Pro Tips

For Complex Visits

  • State chief complaint early and clearly
  • Summarize past medical history briefly at start
  • Number your assessment items: "First, the diabetes... Second, the hypertension..."

For Procedure Notes

  • Narrate key steps: "Injecting 2mL of lidocaine"
  • State consent: "Risks, benefits, and alternatives discussed"
  • Document sterile technique: "Using sterile gloves and drape"

For Time-Based Billing

  • Mention time explicitly: "We've spent 25 minutes discussing treatment options"
  • State complexity: "This is a complex medical decision because..."

Quick Troubleshooting

Poor transcription or note quality?

  • Check device placement
  • Reduce background noise (close doors, windows)
  • Speak slightly louder and clearer

Missing information in notes?

  • Use more specific trigger phrases
  • Add post-visit dictation
  • Verbalize findings more explicitly

Wrong section placement?

  • Verbalize section headers: "For the physical exam..." or "Regarding the plan..."

Remember: The AI is your scribe, not your mind reader. Work out loud for best results!
